"Higher level of care" definition was a little unclear. The definition is "something chronic", yes? The broken wrist/hand example with ortho referral (not chronic) contained copy and deductible costs, yes? If both are true, logical conflict. Please clarify or redefine or refer us to a document with criteria.

So Access Health Clinics are fully staffed facilities, not like some of the Urgent Care's out there that are only staffed by Physician Assistants' with no doctors on site
